Insurance and Billing
Our practice accepts most Dental Insurance plans. As a courtesy, we bill your insurance; however, your portion is required when service is rendered, unless prior financial arrangements are made with one of our Financial Coordinators. Payment can be made by cash, check and most major credit and debit cards. A $15 service fee is accessed to returned checks.

Your insurance is a contract between you, your employer and the insurance company. We are not a party to that contract. YOU are responsible for knowing your policy provisions. Insurance coverage and limitations vary (for example, Composite vs. Amalgam fillings). We do our best to provide insurance estimates for our services; however, ultimately you are responsible for any portion your insurance does not cover. Contractual stipulations of your policy may require a referral form and/or co-pay on the day of your appointment. Contact your insurance to inquire as to such guidelines.

Please provide us with current insurance information at your appointment(s). If you do not have your information with you, please call it in to our Billing Department within 24 hours. This will prevent improper or delayed claim submission. All charges, regardless of coverage, are the patient's responsibility. We ask that you follow-up with your insurance company if a claim is not processed within 45 days.

Medicaid patients must present their Medicaid card at each appointment.
